Douglas "Doogie" Howser (Neil Patrick Harris) ist ein Genie. Er ist 16 Jahre alt, lebt noch bei seinen Eltern David (James B. Sikking), einem Arzt, und Katherine (Belinda Montgomery), und arbeitet selbst als Arzt im Eastman Medical Center in Los Angeles. Dr. Doogie Howser ist der jüngste Arzt im Land. Seine Kollegen, unter ihnen Personalchef Dr. Benjamin Canfield (Lawrence Pressman), Dr. Jack McGuire (Mitchell Anderson), Dr. Ron Welch (Rif Hutton) und Schwester Curly Spaulding (Kathryn Layng), akzeptieren ihn als Mediziner, weisen ihn aber in die Schranken des Teenagers, wenn er allzu erwachsen auftreten will. Doogies bester Freund Vinnie Delpino (Max Casella) wirkt gegen Doogie noch unterbelichteter, als er ohnehin schon ist. Später ziehen die beiden in eine gemeinsame WG. Wanda Plenn (Lisa Dean Ryan) ist Doogies und Janine Stewart (Lucy Boryer) Vinnies Freundin. Als Wanda aufs College geht, zieht es Doogie zu Schwester Michele Faber (Robin Lively) hin. Doogie führt ein elektronisches Tagebuch und hält am Ende jeder Episode seine Erfahrungen im Computer fest.
